CONSENT FOR PASTORAL COUNSELING OF A MINOR

Consent for Pastoral Counseling for an individual under 18 years of age)

By signing below, I hereby affirm and attest that I am the parent or legal guardian of a minor child that will be attending Beach Week 2022. I agree that my child may voluntarily receive pastoral counseling from pastors and other Church staff and spiritual leaders (“pastoral counselors”) while at Beach Week 2022. I acknowledge and understand that such pastoral counseling consists of and is limited to spiritual guidance and encouragement pertaining to religious ideals, beliefs and scripture as interpreted and ministered by the above-named pastoral counselors.

I acknowledge and understand that in providing this counseling, the pastoral counselors are exempt from applicable state laws regulating professional counselors and similar professions, such as marriage and family therapists. In most cases, the individual or individuals who provide pastoral counseling through the Church are not professionally trained, nor registered, licensed, or certified by the State of Texas or a national certifying body. However, should an individual providing Pastoral Counseling also happen to be a registered, licensed or certified mental health professional, the Pastoral Counseling provided by such person through the Church is not being provided in that person’s capacity as a licensed professional.

I also agree to fully release and hold harmless the church, its directors, officers, pastors, employees, agents, borrowed servants, contractors, and other workers, paid or volunteer, or their heirs and assigns (“church parties”), from any and all injuries, claims, liabilities or causes of action which may arise from the participation of my child in pastoral counseling during Beach Week 2022.
